Advancing Whole Child Care: InCK Innovations Across Three States [New Jersey, North Carolina, and New York]

Webinar – For Community-Based Organizations, Family Partners, Healthcare Providers

The Integrated Care for Kids (InCK) model was launched to provide states and local
partners with the opportunities and tools needed to better coordinate care and address the
physical, behavioral, and social needs of children and families enrolled in Medicaid and the
Children’s Health Insurance Program (CHIP). The model aims to help kids stay healthy,
supported, and at home.

InCK supports schools, pediatricians, behavioral health providers, and community
organizations in creating a coordinated system of care for children and their families. Using
a whole-child approach, the model aims to reduce unnecessary emergency department
visits, hospitalizations, and out-of-home placements while delivering more efficient care.
The InCK model will continue through December 31, 2026.
