The NJ InCK program is designed to support children with Medicaid residing in
Monmouth and Ocean Counties, ages birth through 20, who may have medical and
social health care needs by helping families find and access community-based
resources. These resources may include housing assistance, food pantries, utility
support, school‑based services, legal services, and other community supports.
Please join us for a webinar exploring the importance of Whole Child Care and
Family-Centered Care through the NJ InCK model. Participants will learn key
lessons from the NJ InCK team, drawing on more than six years of integrating this
model into primary care pediatrics and community‑based systems. The webinar will
feature the Principal Investigator of the NJ InCK program providing an overview and
background, a Family Support Specialist sharing lived experiences, alongside a
Pediatric Physician Champion discussing the benefits and challenges of
implementing this care model in a pediatric practice.
