We would love your involvement!

Get Involved for Families and Youth

If you are a parent or guardian with a child who lives in Monmouth or Ocean county, ages birth through 20, enrolled in NJ FamilyCare, you are eligible to take the NJ InCK HealthStory to unlock your child’s circle of care.

We encourage you to join the NJ InCK journey by:

  • Completing the NJ InCK Interest Form
  • Once verified, taking the NJ InCK HealthStory within 5 business days
  • Unlocking your child’s circle of care, potentially including an advanced care management team: social worker, child life specialist, community health worker, family support specialist
  • Reviewing the NJ InCK HealthStory results with your child’s doctor

If you’re a healthcare provider in Monmouth or Ocean county serving children and families enrolled in NJ FamilyCare, you are automatically enrolled in the NJ InCK program as a healthcare partner.

If you’re a community-based organization in Monmouth or Ocean county serving children and families enrolled in NJ FamilyCare, we invite you to join our circle of care.

We encourage you to join the NJ InCK journey by:

  • Joining the Partnership Council
  • Helping the children and families you serve share their NJ InCK HealthStory
  • Working with us regarding additional services we can offer our families
