How it Works

① Find out if NJ InCK is right for you

NJ InCK is available to children, aged birth through 20, living in Monmouth or Ocean counties enrolled in NJ FamilyCare.

Click below to complete the NJ InCK Interest Form in English or Spanish.

② Take the NJ InCK HealthStory

• Once verified, you will receive an invitation link within 5 business days via email or cell phone to take the NJ InCK HealthStory (PLEASE CHECK YOUR SPAM FOLDER). 
• Click the link and enter your email or cell phone number to sign in. 
• A temporary code will be sent to your device. 
• Enter the code to begin. 
• When finished, press submit.

③ Connect with NJ InCK

A NJ InCK care manager will review the submitted NJ InCK HealthStory.  Based on the results, our care manager will contact you to create a personalized care plan for your child.  Your child may have access to a support team including a social worker, community health worker, child life specialist and family support specialist.  The HealthStory results will be shared with your child’s primary care doctor.  You can talk to your child’s doctor about the results at your next visit.
